USS Cushing Association Membership and Mailing List Form Please scroll down to the Form. There is an Annual Membership fee of $10. The dues are assessed yearly from February to February. Please send your Membership Payment to: Paul Frisco 11 Blossom Lane E Middletown, NJ 07748 Along with your check, please include the following information: Your Name Your rate or rank while serving aboard a Cushing Which Cushing you served on When you served aboard a Cushing A good mailing address for the Newsletter and Reunion information Your phone number if you wish Your e-mail address name Your name will be entered into the Membership Roster without the dues being paid. You will NOT recieve the Association Newsletter, nor the reunion notices if you have not paid your Membership Dues. For more info on the dues, click here The information gathered here will be placed in the membership roster. The roster is printed once per year and is sold only to members of the Association. The price of the roster covers the printing costs. If you are interested in purchasing a copy of the current roster, contact me. Your address also ensures that you will receive the Association's annual newsletter and each year's reunion announcement, if your dues for the current year are paid. This form is also used to gather information for the on-line Mailing List.

1. Effective immediately, there is an Annual Membership Due of $10.
2. When a name is put on the Full Roster, that name will remain on the Roster until the Member or the Member's survivor asks that it be removed.
3. All Members and Associate Members are asked to send their check for $10 to

   Paul is the Treasurer of the Association, and will keep track of who has paid their Membership Dues. He can be reached by clicking here.
4. Each year's dues are to be mailed to Paul Frisco. Dues are assessed yearly, each February.
5. The Membership Dues have become necessary due to the cost of mailing out Reunion notices, the Newsleter, and other related matters.
6. Reunion notices, Newsletters, and other matters will only be sent to those members who have paid their yearly dues.
7. When sending your dues in to Paul, please include the following information
   Your complete mailing address
   Your ship's hull number and era (i.e. WW2, Korea)
   The years you were aboard
   Your rate or rank
   Your phone number
   Your e-mail address
